LATERAL CANTHOPEXY IN DOWN¢¥S SYNDROME EXTERNAL VERSUS TRANSCONJUNCTIVAL APPROACH



Abstract
Down¢¥s syndrome patients possess multiple facial stigmata of their chromosomal abnormality. Surgical mitigation of some of these may benefit both the patient and family, and may influence the patient¢¥s aceptance by the general public.
The four ocular abnormalities frequently present in Down¢¥s syndrome patients are high palpebral fissure inclination, short palpebral fissure, wide intercanthal distance and epicanthal high inclination of palpebral fissure.
The anatomic characteristics of lateral canthi in Down¢¥s syndrome are shortage of skin and orbicularis oculi muscle of upper lid, and higher location of lateral canthal mechanism.
The external approach may provide wide exposure, but it leaves external scar and has possible injury to ocular globe. We used external approach and transconjunctival approach to solve this prolems. The advantages of transconjunctival approach is to nonstigmatize the Down¢¥s syndrome because of absence of scar. But, it has limited exposure and possible injury to lateral rectus muscle.
Lateral canthopexy in Down¢¥s syndrome has additional effect of lengthening of palpebral fissure. Simultaneous augmentation rhinoplasty was performed to improve the epicanthal fold and restore the nasal contour.
The patients have been followed up over 1 year to 3 yesra.
